Mental health and substance use challenges can range from mild disruptions to crises that threaten daily life. Not everyone needs full hospitalization, but instead many need more support than weekly outpatient therapy can provide. This is where Partial Hospitalization Programs (PHPs) come in.

PHP is a structured, intensive treatment program that provides a day-long therapeutic environment while allowing patients to return home at night.

Think of it as the bridge between inpatient care and outpatient therapy – a program designed to stabilize symptoms, teach coping skills, and set patients up for long-term recovery.

The term partial hospitalization can be confusing, as people think they will spend an initial time in hospital, but that is not how this works, as we will now explain.

What Is A PHP?

A Partial Hospitalization Program is sometimes called a “day program” because participants attend treatment during the day and live at home in the evenings. It is more intensive than outpatient therapy but less restrictive than full inpatient hospitalization.

PHP is appropriate for individuals dealing with:

  • Mental health disorders like depression, anxiety, or bipolar disorder.
  • Substance use disorders requiring structured detoxification support.
  • Co-occurring conditions, where both mental health and substance use issues are present.

The goal is to provide highly structured, therapeutic support while allowing patients to maintain some independence in daily life. In effect, it is a halfway house between daily counselling and support and the rigors of daily hospitalization – giving you more support but not to the level of full admission.

PHP is particularly effective for individuals with serious mental health challenges that disrupt daily functioning. Conditions such as major depression, anxiety disorders, bipolar disorder, post-traumatic stress disorder (PTSD), or severe stress responses often require more than weekly therapy.

Patients may struggle with intense emotional swings, intrusive thoughts, or overwhelming anxiety, which can make daily tasks difficult. PHP provides consistent structure, therapeutic support, and skill-building to help patients manage symptoms, regulate emotions, and develop strategies for coping in real-world situations.

How PHP Is Typically Structured

PHP is highly structured, usually running 4-6 hours per day, 5 days a week. The program blends several elements to create a comprehensive treatment experience.

  • Patients meet with licensed therapists for one-on-one sessions
  • Psychiatric evaluations and medication management
  • Personalized treatment planning tailored to each patient’s challenges and goals

2. Group Therapy:

Therapies Used In PHP

PHP programs rely heavily on evidence-based therapies, such as:

  • Dialectical Behavior Therapy (DBT): Focuses on emotion regulation and coping skills
  • Motivational Interviewing (MI): Supports patients in finding their internal motivation to change

Dialectical Behavior Therapy is becoming increasingly useful, although it is little known outside of specialist settings.

It’s a type of cognitive-behavioral therapy (CBT) designed to help people manage intense emotions, self-harming behaviors, and chronic stress. It combines traditional strategies for change with mindfulness and acceptance skills, aiming to balance two forces: accepting yourself as you are while working to improve harmful behaviors.

DBT focuses on four key skill areas: emotion regulation, distress tolerance, interpersonal effectiveness, and mindfulness.

Treatment usually includes individual therapy, skills training groups, phone coaching for crises, and support for therapists through consultation teams. DBT has been shown to reduce self-harm, suicidal behaviors, and hospitalizations while improving emotional control and overall quality of life, which is why we now have two fully trained DBT therapists on staff.

Goals And Outcomes Of PHP

PHP is designed to stabilize, educate, and empower. Key objectives include:

  • Stabilizing symptoms: Emotional, behavioral, or substance-related
  • Teaching coping skills: Practical tools for managing stress and triggers
  • Preventing hospitalization: Reducing the risk of crisis
  • Medication management support: Ensuring proper treatment adherence
  • Planning for aftercare: Preparing patients for ongoing recovery and outpatient therapy

Completion of PHP often positions individuals for successful transitions to less intensive programs, such as IOP (inpatient – outpatient) or full outpatient therapy.

Duration and Flexibility

PHP programs typically last 4-6 weeks, though length varies based on patient progress. The program is flexible, allowing patients to:

  • Step up from outpatient care if symptoms worsen
  • Step down from inpatient care to gradually regain independence

Treatment intensity is adjusted based on individual needs and therapeutic progress, and is completely tailored to you as an individual and not a cookie-cutter structure that you will feel trapped and lost within.

PHP experiences are intensive and structured, but ultimately rewarding.

Patients will follow a consistent daily schedule that includes therapy, education, and wellness activities in both individual and group sessions.

Peer interaction offers social support and increases a sense of accountability with others and those supporting you.

One of the main challenges can be adjusting to the program’s intensity, but most patients benefit from the structured environment. Remember, as an individual, you will be monitored and can use your voice to adjust the intensity and pitch of the program at any time.

Outcomes vary by individual, but many find PHP transformative in stabilizing symptoms and building life skills.

The Care Team

  • Recovery specialists

Teams collaborate to create individualized treatment plans and adapt them as patients progress.

Recovery specialists are professionals who provide hands-on support for daily recovery tasks and long-term goal setting. They work closely with patients to help them navigate treatment plans, identify triggers and develop coping strategies, and practice coping strategies in real-world situations.

They also connect patients with community resources, support groups, and vocational or educational opportunities, helping ensure recovery continues outside the PHP environment. Essentially, they act as guides and advocates, keeping patients accountable and empowered throughout their recovery journey.

Insurance And Access

Most programs can be accessed with both commercial insurance and Medicaid/Medicare (although this varies by location provider).

Your program may also provide travel assistance to and from the location and coordinate care with outpatient providers, making participation more feasible.
